Looking for a provider?
Find the name of your administrator on the back of your Plan ID card and follow the instructions or links.
Medical benefits and your medical plan administrator is:
Aetna (Premier, Saver, and Contribution)
• aetna.com/individuals-families/find-a-doctor.html
• After entering in your location, select the Aetna Choice POS II (Open Access) Plan.
• If you have issues locating a provider, contact Aetna at 855-548-2387.
Aetna (PPO)
• Go to aetna.com.
• Select “Find a Doctor”
• Under “Guests” select “Plan from an Employer”
• To find providers on your plan, enter your location and select “Search”
• Scroll to find “Aetna Open Access Plans” then select “Aetna Choice POS II”
• Then search by provider name, specialty, or category

Pharmacy and your pharmacy benefit manager is:
• OptumRx: optumrx.com/public/landing
– Associates enrolled in the Premier, Contribution, or Saver Plan option, use the following links to determine your
pharmacy providers:
• If your work location is five miles or less from a Walmar or Sam’s Club pharmacy, you must use any Walmar or
Sam’s Club pharmacy (including in-store or in-club), Walmar Home Delivery Pharmacy, or OptumRx Mail-Order
Pharmacy for generic and brand-name drugs.
• If your work location is more than five miles from a Walmar or Sam’s Club pharmacy, you may use any OptumRx
network pharmacy (including any Walmar or Sam’s Club pharmacy), Walmar Home Delivery Pharmacy, or
OptumRx Mail-Order Pharmacy for generic and brand-name drugs.
• Specialty drugs must be filled through Walmar Specialty Pharmacy or Optum Specialty Pharmacy.
– Associates enrolled in a local plan option:
• You must use any Walmar or Sam’s Club pharmacy, Walmar Home Delivery Pharmacy, or OptumRx Mail-Order

Networks for specific plan benefits
• Centers of Excellence (COE) – the Plan has contracted with cerain facilities for the provision of cerain services
under the COE program. Paricipation in the COE program must be arranged by the COE administrators listed below.
You may not contact these facilities directly to receive benefits under the COE program. You must work with the
appropriate COE administrator listed below. When receiving services through the COE program, providers of COE
services at that facility will be network providers for purposes of the COE program. The network status of providers of
non-COE services at a COE facility will be determined by your regular medical third-pary administrator network.

• Doctor On Demand – Doctor On Demand is a telehealth service that provides access to medical providers, including
therapists. Any provider accessed through Doctor On Demand is a network provider. To locate available providers at
your location, including therapists, log on to Doctor On Demand and create an account.
• Associate Executive Physical Program – Vice Presidents and above are eligible for physical exams through the
Executive Physical Program. You will find a list of network facilities in the Executive Physical Program. For purposes
of the Executive Physical Program, every provider at the paricipating clinics is a network provider with respect to
services received through the Executive Physical Program. If you are a Walmar Inc. officer, you may confirm your
eligibility for the Program by emailing AEPPbox@walmar.com.
